One of the most important conversations you can have with those close to you is about what kind of care you would want if you should become very ill and unable speak for yourself. These conversations relieve your loved ones of the burden of making healthcare decisions on your behalf without knowing your wishes. 

This workshop is for all adults and will: 

• describe Advance Care Planning using a variety of print and video resources 

• discuss what you need to do in order to have someone make healthcare decisions for you 

• explain what is involved in making a Representation Agreement 

• explain the process of documenting your goals, values and beliefs 

• provide an opportunity to ask questions of skilled Advance Care Planning facilitators
