The two documents attached form part of a plan for seniors to ensure that the hospital has the correct information about them if they need to be admitted. The explanation form will help guide you through the form - and the form itself is important for the hospital - and can save considerable time answering questions on admission. It is recommended that this form be taped to the inside of your home's door for quick access by paramedics or family members.
